Symbols in libknot/internal/ should not have knot_ prefix unless
they are a replacement for a system function.

Although RFC6895 defines error code space to be common for all
response errors, one of them is defined twice (BADVERS / BADSIG).
Also, only Extended RCODE from OPT RR is interpreted together with
RCODE from Header as one number. All other "RCODEs" are separate
error codes with dedicated fields (in TSIG or TKEY RRs).

The config.h is now implicitly included using -include option of the
compiler. After this change, config.h need not be placed at the first
position in all source files.

Not much refactoring done, just that the algorithm name must be
known in advance, so a correct RDATA length can be calculated.

Move (and sometimes add) #include <config.h> at the top of
all *.c files.

Remove all #include ["<]config.h[">] from headers files.
