Extended tests for PKCS #11 key store

The tests will fail due to a bugs in the current GnuTLS. the patches fixing
these problems were submitted upstream.

The problematic tests are run only conditionally when SoftHSM 2 is available.

Fixes #449 by calling the logging function once for the query packet and once for the response packet. This is a speciality just in this module.

Adds RR set redundancy checking to change set RR set additions and
removals, thus cleaning up DDNS a bit. Cleans up and reorganizes the
change set structure and moves code that does not belong there.

It also has the effect changeset_merge produces sane changesets
(non-redundant data) which is necessary for lmdb-based journal.

dname: replace free with mm_free where it belongs
